An 11-year-old male patient was admitted to the hematology clinic for follow-up and treatment with a diagnosis of aplastic anemia. Detailed clinical evaluations, laboratory tests, and bone marrow analyses performed upon admission revealed insufficient hematopoietic cell production in the bone marrow, leading to severe bone marrow failure.

Considering the course of the disease and the current clinical condition, the patient's need for advanced treatment was assessed by a multidisciplinary team. As part of the treatment plan, detailed tissue compatibility screenings were performed on family members to find a perfectly matched donor. Since no perfectly matched donor was found within the family, the process continued through national bone marrow donor banks, and comprehensive donor searches were conducted in relevant databases. However, despite all screenings and evaluations, a perfectly matched volunteer donor could not be found for the patient.

Due to the progressive nature of the patient's clinical condition and the continued need for a transplant, alternative treatment options were evaluated, and the transplantation team decided that haploidentical hematopoietic stem cell transplantation was the appropriate treatment approach. Immunological and tissue compatibility analyses revealed that the patient's father was a suitable donor with 50% tissue compatibility. Following the completion of the necessary preparations, the patient successfully underwent a haploidentical bone marrow transplant using hematopoietic stem cells obtained from his father. The transplantation process was carried out using a multidisciplinary approach in coordination with relevant specialist units, and the patient was placed on a close clinical follow-up program after transplantation. Throughout the process, infection prophylaxis, supportive treatments, immunosuppressive therapy management, and regular hematological evaluations were meticulously maintained.
